Community-Based Therapeutic Services
We literally meet you where you’re at. Community-Based therapy is a convenient option for families who might find it difficult to assemble in the office.
Individual and Family Therapy
Children and families should be seen in context. Lorenz Clinic’s Community-Based therapy can provide services to the individuals as well as to families. At times, both of these options are necessary to provide a holistic approach for clients.
Therapy is often most effective when we can see the problem right in front of us and not in an artificial environment. Clinicians work closely with caregivers to improve their ability to respond to and support their child in a way that promotes improved functioning at home, school, and in the community, while preventing placement in a higher level of care such as hospitalization or partial hospitalization.
Children/families who are enrolled in Medical Assistance or Medicaid programs may have eligibility for receiving services in their homes and communities. Once added to the waitlist, our team will contact you when a clinician is available to provide a diagnostic assessment. This assessment will assist us to determine if community-based services are appropriate and medically necessary. From there, goals will be made in collaboration with the child, caregivers, and the clinician, and the frequency of the therapy and/or skills training service will be determined based on need. Community-based services are generally recommended when behaviors are more frequently and severely displayed in the home.
Referral Process
If you are a professional interested in referring someone to our Community-Based program, please submit our online referral form or print and fax the request to (952) 443.4604 with a signed release of information form. Children’s Intensive Behavioral Health Services (CIBHS)
CIBHS’s unique focus is to provide stabilizing services and support for permanency through maintaining home placement, reunification, transfer of legal guardianship, or adoption. CIBHS will seek to reduce stress and build resourcing for children and families impacted by current stressors and/or their experience in the foster care system.
Lorenz Clinic is one of 12 agencies certified to provide CIBHS to the communities we serve. This a paid service offered through Minnesota Health Care Programs (MHCP) and the Minnesota Department of Health. This service is available to children and families who meet the threshold for medical necessity, and whose current placement is experiencing some risk. Results of collaborative relationships can help the child/youth maintain stronger family connections while in home settings and reduce trauma, stress and anxiety for the child/youth. Services can take place in person (in home, school, clinic, and other community locations) or via telehealth.
Accessing CIBHS
Lorenz Clinic clinicians, dependent on availability, may offer CIBHS services in: Anoka, Brown, Carver, Dakota, Goodhue, Hennepin, LeSeur, McLeod, Nicollet, Ramsey, Rice, Scott, and Sibley counties. The Clinic is currently collecting referrals while also adding potential clients to the waitlist. To join the waitlist, submit a Professional Referral Form.
