First Appointments

Dependent on the type of appointment, there are varying requirements before attending the first appointment with a Lorenz clinician. See below for information about preparing for your first visit.

Psychotherapy Appointments: In Clinic

 

  • Check in online via your patient portal up to 7 days before your scheduled appointment to:
    • sign required consent forms
    • complete the health history questionnaires
    • Verify your information is correct (phone number, address, insurance, etc.)
    • Pay your copay (if applicable)
  • Share your records so your provider is prepared to meet your needs. Primary care, previous prescribers, and any other pertinent information regarding the care you are seeking with our clinic.
    • Records can be faxed to our clinic a 952-361-5511
    • Records can be sent as an attachment via the patient portal
    • Bring them to your 1st appointment
  • Minors 14 and under are not required to attend the 1st appointment with their legal guardian(s)
  • If there is someone or agency you would like your provider to connect with, please visit our website at lorenzclinic.com/resources to complete a Release of Patient Health Information.
  • Copays are due at the time of service. We accept cash, check, or all major credit cards (​​Amex, Visa, MasterCard, or Discover).
  • If you are not using your insurance, payment is due at the time of service. We accept cash, check, or all major credit cards.
  • Please arrive at least 15 minutes prior to your scheduled appointment time and check in at the front desk. If no one is at the front desk, please have a seat and your provider will be right with you.
  • If you have any questions, please contact us at 952-443-4600, Option 1 to speak to a clinic assistant.

    • Telehealth: All minors must attend the 1st appointment with their legal guardian via Zoom on a device that has audio and visual capabilities

    • Mail them to our clinic
  • All minors must attend the 1st appointment with their legal guardian via Zoom on a device that has audio and visual capabilities.
  • You must be in the state of Minnesota for your telehealth appointment
